You might want to tell us about any delays to the referral, if the referral was lost or rejected, and / or whether you had to chase anyone up for information about the referral. You may also want to tell us about the communication given, any support given to you, etc. You might want to tell us about any positive and negative impacts on your ability to do your day-to-day activities, your ability to work, your emotional wellbeing, your relationships, etc.

Your answers will help NHS and care services. The information you share with us helps us spot trends to identify areas for improvement. We may use quotes in our reports, but we will not use any information that will identify you. We will log your feedback and share what you tell us with the organisations who make decisions about local health and care services. We take out any information that could identify you when we do this.

As well as your feedback, we also ask you to volunteer some personal information. This helps us to understand how different groups experience local health and social care services and supports our focus on improving equality, diversity and inclusion.

If you are sharing information on behalf of another person, make sure that you have their permission to do so, or the information you do share should be anonymous.
